Typically code requires fire extinguishers to be mounted not higher than 48" to the center of the handle from the finished floor.  Please check with your local codes to clarify this requirement. 

Handicap toilet stalls with a floor mounted toilet are required to be a minimum of 60" wide clear inside and 59" deep clear inside. For a wall mounted toilet there needs to be 56" clear front to back.  The door opening has to have a minimum of 32" clear space; therefore, you will need to use a 34" or 36" door on this stall.  A crutch or ambulatory stall requires a clear space of 36" wide with 60" deep.  Again, the ambulatory stall must have  32" of clear space. Please check with your local codes to verify their requirements. 

For adult toilets per ADA requirements the mounting height to the center of the grab bar for both the side and back grab bars is between 33" for the bottom of the grab bar and up to 36" for the top of the grab bar.  For the 18" vertical grab bar ADA requires this bar to be mounted 39"-41" to the center of the grab bar from the back wall and 39" to 41" from the floor to the bottom of the grab bar. However, always check with your local codes to determine the exact placement.
